What Wellness / Nursing does

Wellness support includes RN-led check-ins and care-related communication based on the participant's support needs. The service is designed to support communication and consistency, not to replace emergency or primary medical care.

Visit frequency

Visit frequency may be once a week, multiple times a week, every other week, or based on participant needs, tier level, and approved schedule.

Funding
DDSi accepts Indiana Medicaid waivers, including the Community Integration and Habilitation Waiver (CIH), Family Supports Waiver (FSW), and Traumatic Brain Injury Waiver (TBI). The Health and Wellness Waiver (H&W) is coming soon.
